How do you differentiate math instruction for 4th graders?

I've used differentiated instruction in my math classroom before. However, I feel like I still have a lot to learn. How often do you differentiate instruction in your classroom? Also, how do you chose or create the activities? Are there other adults who help you, or do you do this on your own? Do your students rotate through the same type of activities each time, or do the activities change each time? Finally, any helpful hints to make preparation for differentiated instruction quicker / easier? (It takes me so long to prepare lessons in this way!)
