How do you establish and maintain an open culture that encourgages candor and discourages concealment? ?

Cookies and milk.  Everyone loves talking while standing around a table filled with cookies and milk.  Or open up a bar.  Can't get people to shut up in a bar.

It wasn't clear whether you meant to create this culture in an office or in a relationship. Either way, I think the best way to create this would be to show it your self. If people around you realize that these are your qualities, it would also enable them to be be at ease around you and be honest with their feelings. Keep in mind that what you will receive in return may not always be music to your ears! Some people may use this also to be callous in their approach.
