How do you fill out an Industial User Discharge permit for San Diego?

I need help filing a permit for an industrial user discharge of waste water. I have no idea about the cooling towers, reverse osmosis, steam loss, evaporation, etc... Can someone please help...maybe a link to where there is explanations of page 2 of the document. the permit can be found on this link.

What type of business do you have? Do you have cooling towers or any of that stuff.
