  2. You can see them favoring their sore foot. This is especially evident at a trot. There is also a distinctive head bob when the sore foot hits the ground.

I'm hoping I'm understanding your question correctly.  If a horse is lame in the front, while walking his head will go up.  As his head goes up the hoof in the front that just touched the ground is where the soreness is.  If a horse is lame in the rear its opposite.  The horses head will actually lower instead of raising his head.  The hoof that is stricking the ground at that time is where the soreness is.  Hope I helped and sometimes soreness is not always eveident like that.  Experience and a good eye is what it takes alot of time.
