You are correct in that you can see bedbugs, adults are typically about 3/16 inch to 1/5 inch long.

Bedbugs are a pest of expsoure and are most frequently found in dwellings with a high rate of occupant turnover, such as hotels, motels, hostels, dormitories, shelters, apartment complexes, tenements, and prisons.

Thus, generally, to have them in your property either yourself or someone who has come in to your house, has been in previous contact with them.  To have them is not necessarily a sign of bad hygiene/cleanliness at all.
