  1. You can't. That is an indicator that the seal has broken on the window and the gas has escaped allowing air and moisture to get between the panes. You need to get a window person over to replace the gas and seals after drying it out or you will get mold.

  2. the seal failed but you can use a single edge blade and slice along the edge of the glass to open up the insulated unit.  Use gloves and do all 4 sides.  Don't use a utility knife as that blade is too thick.  Once you have separated the piece of glass from the spacer you use a high quality glass cleaner and clean the inside of the glass.  Put the piece back on top of the insulated unit and smear silicon around the edges of the insulated unit.  Use a putty knife. Will get a couple more years out of the unit if it hasn't had the moisture inside of it for very long.  There is always the possiblity of changing the desicant inside of the metal spacer, unless it is a "swiggle" type space (which is c**p).  This is labor intensive but do-able.  You always have the possiblity of breaking the piece too.  but you can always go to a glass shop and buy the individual pane.  Some may think to just pay someone to replace the piece but if you have more then one then it gets VERY expensive.

  3. There is only one solution, and that is to replace the glass.  Not sure what type of window or if under warranty, but you may have to buy the entire part of the operating window(sash) if necessary.  In vinyl windows, you can just replace the actual glass and not the entire sash.  good luck.

  4. The pane has to be replaced. The factory injects an inert gas between the panes before sealing.  Any air that is between the pane will contain moisture an will cause condensation.
