How do you handle a sister that is a prositute and she leave her kids everywhere she is 125 pounds and ?

You don't.  But I'd look out for her kids.  Its not their fault she's the way she is.

Call child protective services, see if a family member is able to care for them and get custody changed to someone who will have them in a stable safe environment. Make sure you have proof or people willing to attest to the fact that she is an unfit mother.
