  1. If you mean the slipknot with which you start a knitting project, I don't knit mine.  I use my fingers to make one.  Wrap the yarn once around two fingers, bring it under those fingers, spread them a little, then grab the yarn with those same two fingers and bring it up through the loop you made by wrapping the yarn around those two fingers.

If you mean somehow knitting a slipknot in the middle of the project, please doublecheck that you have the right word.  I haven't heard of knitting a slipknot in the middle of something.  Once you have the right word (slipped stitch, some other kind of knot, what have you) go to www.lionbrand.com or www.coatsandclark.com and look up their tutorials.  They're free.

  2. when you make the slip knot, have it a little loose so that there is enough room for both needles and then when you come back to that slip knot just knit like you would normally.

  3. If you have difficulty knitting into the slip knot, then cast on one more stitch than is called for and knit across to the slip knot then simply remove it and pull it out.  All that will happen is you'll have a little bit more tail length, but nothing will happen to your knitting.  It will not unravel.  That said, as others have pointed out, don't pull the knot too tightly.  Your stitches should hug the needles, not strangle them.

  4. Don't pull the yarn too taut when you make it. Count it as the first stitch when you cast on.
