If you are having trouble focusing, or fine print is difficult or impossible to read, you most likely need a change in your prescription.

You need an eye exam every year. or more often if you are having headaches or eyestrain.

By the sharpness of the images that you see. If they are clear and crisp, both near and far, they are fine. If the images you see are not sharp, glasses could be either too strong or too weak, but it is very unusual for them to be too strong unless you bought magnification glasses without a prescription.
