How do you make/get an icon for yahoo answers?

Go to your Yahoo! Answers profile and click the "Edit my Info" link.  You should see a section Picture where the last option is "Use my Yahoo! Avatar".  Below that should be a link to edit your avatar.  Click the link, customize your avatar to look how you want, then save it.  Make sure the "Use my Yahoo! Avatar" option is selected and click the Preview button at the bottom of the page.  Then click OK and you should be all set.
