or if only needing it slightly warm: place milk in a small metal bowl & place that bowl in a larger bowl, pour hot water around the smaller bowl.

You can use a double boiler by boiling water in one pot and put another pot on top filled with milk. Just make sure you constantly stir the milk so that it doesn't scald.
