How do you make "intezaar" in past form??

This can't be converted into past form.....but when u use it in a sentence u can convert the sentence into past form but not the word....

for example: maine intezaar kiya tha...

meaning: I waited

There is no past form. You woud just say "meine intazaar kiya".
