How do you notarize a consent letter for permission to issue a passport for a child under 14?

the father cannot be present at the appointment and must therefore issue consent that is notarized

  1. Let me add to the above procedure, which is correct, that you can find people to notarize a letter at most banks, and/or post offices. Just make a call to find out if they  have someone who can notarize the letter. Good Luck.

    Edit: Just FYI: To add to Charlie's below, which is also correct, if you are taking a minor from the US who is not yours, you also need a notarized letter from their parents (we did this when we took my son's girlfriend and my daughter's boyfriend outside the country). Some countries will ask to see that too.

Later, after the child has the passport, if the child will be travelling outside the U.S. without both parents, the absent parent needs to write a letter of permision  for the child to travel also...and it must also be notarized.  Some countries require this and will ask to see it . Others will not, but it is safest to have documentation that the child has permission of both parents to leave the U.S.

  3. go to any notary after you have written the letter do not sign or date the letter until you are in front of the notary . then sign and date the letter in front of the notary let them put their seal on it and sign it . thats all there is to it
