  1. most commonly, your physician (and you) will have to show your history of back pain. additionally, most insurances will require that you attempt to lose weight to see if that cures the problem..once everything is tried and failed, generally is when you would start the pre-authorization process. before you get anything done, make sure you have it in WRITING that it is approved by the insurance company. additionally, i would also call your insurance's pre-auth department and ask what their guidelines are for that specific procedure just to make sure you understand and your physician didnt leave out any important details.

  3. You get a letter from your doctor and or chiropractor about how much pain you are in from the "excess bagage" and they should know how to explain it to the insurance company.

  4. The PS you are consulting with will send pictures and his diagnosis to the insurance company.

  5. It depends on the company. There are different standards for it. Some might make you go through physical therapy or go to several different doctors first. And some won't cover it at all, claiming that it's 'cosmetic' (consider yourself lucky if you don't have one of those).
