  1. You have to have an established GHIN # which will show your 1.4 index.  That will allow you to compete in the qualifying rounds, plus pay the entry which is $150.  But the main thing is having that GHIN #.  You can learn more at http://www.ghin.com and you can also read on the qualifing information for the us open at :

  2. Sign up and play well, very well.  You have to have a certain handicap to even attempt.  There is local qualifying played over 18 holes.  Then there is regional qualifying played over 36 holes.

  3. Well, first of all you have to have an established handicap of 1.4 or better (lower).  You might be asked to "prove" your worthiness by submitting attested scorecards from tournaments and such, not just a 1.4 handicap at your local course.

    Assuming that you are not a professional golfer, you will have to qualify through a local and sectional qualifier.  If you do well enough in those, you are in the tournament.

    The qualifiers can be very intense and the sectional usually include professionals who don't automatically make it into the tournament.
