Try some hydrogen peroxide and ammonia.

Did you try hot bleach?  If not then you may have to pressure wash to sandblast a layer of the sidewalk off.

Pour some peroxide on it give a little scrub and rinse with water.

...ammonia, and if it doesn't work, I'd try Oxyclean...
