Replacing Your Placard

Should you ever misplace your placard, you can fill out an MV-145A form to request a replacement. If in the meantime you find your placard, you are required to return it to the BMV.

Read both applications carefully. Not only do they require your signature, but depending on your situation, they might also need the signature of either a certified physician or a police officer, and either must be notarized.

a certified recognized doctor to get your permit renewed.

Note: If it's a permanent they will automatically send it to you.

If it's temporary then your doctor will have to fill another form out that you will have to take in with the "application for disabled..." form that you have to fill out.  It's very simple.
