Find all pages. Then go to Merlin's House in Hollow Baston and access the book. Complete all the minigames with a CLEAR at the end. That should complete 100 Acre Woods and repair the book. This also unlocks a keyhole. Remember your playing KH2, Sora's objective in the worlds is to unlock keyholds to create paths to other worlds.

Unless you're talking about the first KH, then Sora will be locking keyholes. But for the pages just follow the same steps as I said at the top, except Merlin's House is in Transverse Town.

Hollow Bastion - Crystal Fissure

Agrabah - Ruined Chamber (after Magic Carpet)

Land of Dragons - Throne Room

Pride Lands - Oasis

Disney Castle - Library

Locations of Torn Pages (KH): [I took this straight from my source]

-Agrabah: Cave of Wonders: Dark Chamber

     It is in a treasure chest in the middle island. Open the chest to receive your first Torn Page.

-Monstro: Chamber 6 [*]

     On top of a platform on the side of the chamber. This platform is too high to reach with jump, so throw a barrel on the top step. Then jump onto the barrel and from there jump up to the platform to open the chest.  (You can also Superglide over from the opposite ledge if you have it learned).

-Atlantica: Ariel's Grotto

    Go into Ariel's Grotto and simply open all the treasure chests. It should be near the top.    

-Halloween Town: Research Lab

    Search the bookcase to find this Torn Page.

-Total of 51 Dalmations [*]

    After finding a total of 51+ Dalmation pups (17+ chests' worth), head over to the Piano Room of the Dalmations' House (off of the 2nd District) and approach Pongo & Perdita; they will reward you with a Torn Page.

Well, you have to get all the torn pages for it. You'll be getting them along your journal. Wow, I sound like a game guide. XD Anyway, if you want know where they are....

Traverse Town: Rescue 51 Dalmatians.

Agrabah: Dark Chamber in the Cave of Wonders.

Monstro: Chamber 6.

Halloweentown: The bookcase in the Doctor's Lab.

Atlantica: Ariel's Grotto.
