  1. I merely drop the thread I am crocheting, pick up the new thread on the hook and continue with the pattern.  Two threads are left hanging; you may tie them together, cut them, and weave the ends into the pattern.

  2. Don't completely finish the stitch before you change colors usually with one pull through left (for sts like sc, dc etc).  Use the new color to complete it.  When you pull up a loop in the next st (to start you next st) put the tail in between the working yarn and the loop, or basically lay the tails along the top of the piece this way you crochet the tails in and don't have to weave them in

  3. I like to drop the old color, leave a medium length tail on it and tie the new color to it loosely where the next stitch begins.  Start the rest of your rows, etc, and when done, come back to weave in those two tails with a large eye needle so that it blends. If the knot is too large or distracting, undo it and weave in the tails as thoroughly as possible so hey won't come undone easily.

  4. Start your last stitch before the color change and work it until you have two loops left on your hook.  For example, if you are doing a double crochet: yarn over and draw up a loop, yarn over and pull through first two loops on hook.  Drop the old color and pick up the new.  Yarn over with the new color and pull through remaining loops.  Continue in the new color

With this method, you don't get holes in your fabric like you can if you just drop the old yarn and pick up the new.  This is also the best way to join a second skein if you aren't near the edge when you run out of yarn.

As a note, I strongly recommend against EVER tying knots in your yarn as you work.  It is very easy for them to become visible in your fabric, which can really ruin the way it looks.
