i need standardized questions to measure level of awareness on diabetes... and when i get the results, what statistical treatment should i use??

From my experience, the best way to find out what standardized test/questions to use for doing research, you should look at scholarly research articles that report scientific findings.  When you locate articles in which a researcher studied "level of awareness on diabetes" then you will be able to identify what test/questions they used to do their research (usually mentioned in the methods section of the article).  You could also look at what statistical treatments were used with research participants (also likely in the methods section).  

    Once you have a name or author of a test, your chances of locating it are greatly improved. You can often purchase a copy of the test to use from the publisher (especially when you are wanting standardized questions).
