How do you type a statement saying why my father isn't here for my passport renewal?

I know where to get it notarized, but my mom tells me to type it for her, while she dictate what to write (I translate it in English).

My question is, how can I write such statement? What should I write? How should I set it up?

An American passport?  Are you less than 16 yeras old?  If you are applying for an American passport and you are less than 16, you cannot renew your passport, you must apply as if it is the first time.   Both parents must accompany you .  If one parent cannot be there, he must send a notarized letter giving his permission for you to get the passport.  Neither you nor your mother may write this letter.  Your father must write it, sign it and have it notarized, then send it to your mother to carry with her when you go to the post office to submit your application.  See the site below and read #5.  If you are 16 or 17,  permission of both parents is not necessary.
