  1. A certificate of employment can be a simple letter stating the name of the individual who works for you, and verification that the person is actively employed by your company.

    It should include the name, and address of your company.  It can include the company's contact person, or the employee's supervisor's name, if you wish.  It should be signed by a responsible officer of the company.

    If there is a specific recurring need, your company can create a form that resembles a certificate with fill-in-the-blank spaces for the information needed.
