    You can start with something simple:

    Date________

    Reference:  Purchase of _____ (or something)

    This is your first notice of payment due in the amount of _______ for the _______ purchase (received, loaned) on date ________.

    Please pay amount _____ by _______ to the following address.

    I dont know what it is you need payment for or I could be more specific but honey if I was you, I would go to your friend and curse them out for sending you a dead beat customer.

    you may want to go to the library and get a book on legal forms or business forms.  thats all you need.  its not about being nice or mean, its about business.
