  1. One -- are you sure you need to ask him and only him for guidance?  Usually there is a grad student who does all the grunt work of advising the newbies.  I'll call the office and ask the advice of the Department secretary -- he/she are the power behind the throne!!!  (And be sure to stop by and thank them next time you're on campus.)

If he is the one -- just email him!  Introduce yourself in the opening paragraph (Hello Professor Smart,   My name is Thinker and I am seeking admission to program X.  I would like to schedule an appointment with you to evaluate my coursework.  Do you have any openings during the coming week of July 28th?  I can also provide you copies of my academic record in advance, if that would assist the process.....  You get the idea.

    Good luck!!

  2. Dear Professor ______:

I need to make an appointment with you as soon as possible. I need my college work reviewed, since I am transfering to _________. Please get in contact with me at (a phone number, the E-mail address).

Thank you so much, and I am looking forward to talking with you over this matter with you.

    Sincerely,

  3. Whenever I need to set up an appt with a professor, I start off with,

"(Prof., Dr., whatever their title is) _______,

My name is _____ and I am a ____ major.  I would like to set up an appointment to meet with you about my college coursework.  Could you email me back with your office hours or a good time to meet with you?  It would be greatly appreciated.

    Thank you,

    ________"

Just be very polite and straight foward.
