  1. Quite simply, people would just say

"O-namae wa?" (which literally means "Name?")

("O" is a formal prefix to add to nouns)

    ("namae" is "name")

    ("wa" is a subject marker)

But if you want to make things long-winded to the Japanese, you would say:

"Anata no namae wa nan desu ka?"

(this is the literal translation of "What is your name?)

They would answer: "....(insert name here).... desu." (desu in this case means "am")

  2. "Onamae wa?" (Oh-nah-mah-eh)

or namae wa the O makes it more formal but since japanese are into formality id say use the O.

to reply "my name is": Watashi wa ______ tomoshimasu. insert name in the blank (the above is more formal)

or  just Watashi wa ________ desu.

and the pronunciations are all pretty much how they look. i's sound like ee's  a's like Ah's  o's like oh's ^^

  3. What is your name? - Anato-no namae wa?

  4. Onamae wa?

    Onamae wa nandesu-ka?

    Those are polite way.

    If it's casual situation,

    Namae wa?

    Namae nante iuno?

  5. "what is your name?" = "fujitzu suzuki yamaha?"
