How does HD cable work?

I have just a basic analog cable connection (75 channels) and I recently got a new 32 inch Samsung 720p TV so I want to upgrade to HD cable. Is it expensive? And can it work with an analog connection? What kind of box will I need? How much more per month am I looking at with HD cable?
