  1. Generally a black hole forms when a very large star explodes in a supernova.  I think the star's mass has to be at least 3 or 4 times as large as the sun, or else it will become a white dwarf or a neutron star.

What happens is that the star collapses under its own gravity due to lack of energy.  For example, the sun is currently burning and the internal gravitational force is balanced by the sun's huge amount of heat, which pushes outward.  Howerver, once the star gets burned out and goes supernova, the remains lose most of their energy.  Howerver, they still have gravity pushing them inward.  As a result, no force counteracts the gravity (not even the forces that give structure to particles) and the mass just shrinks until it has zero volume.

  2. useually from the collapse of a really really big star, ultimatly the density gets so high that the escape velocity(speed needed to escape gravitational pull) is higher than the speed of light.

  3. I presume you mean a Black Hole? This is a question better asked in the Astronomy section, but I'll take a swag:

A black hole is a super massive remnant of a huge star which has gone supernova. After consuming most of the gas in conventional fusion reactions, super massive stars "die' with an explosion which bows off the outer gas remnants of the star, the star's remaining material mass then causes it to collapse onto itself. If the stars mass is large enough, the gravity of the star will cause the collapse to progress to the point where gravity is so strong that nothing, not even light can escape the gravitational field. One can think of a black hole as a kind of "drain" where all matter near it "drains" into the singularity.

Most all galaxies have large black holes at their centers, ours (Milky Way) certainly does.

  4. Usually the block will have a small chip, then it grows larger, until it is a complete hole.
