  2. We may be missing the point of the question here.

    A school may issue a new SEVIS I-20 form for another year of studies, but the school may not extend the visa.  Two different things here.

    One can remain in the U.S. studying even though the VISA has expired.  One applies for an EXTENSION OF STAY through CIS at least 90 days before their status will expire.

    The VISA can be renewed at the Embassy or Consulate where the student applied for and received the first one.

  4. A university that wants to accept international students must apply to USCIS for permission to do so.  If that is granted, they are then authorized to issue I-20 forms which foreign students can use to apply for US F-1/Student visas.
