  1. As altitude increases and the higher up you go, the colder it gets.  Also, some cold fronts sometimes have extreme freezing temperatures.  When they run into warm fronts, storms happen and the moisture from warm air quickly freezes, in the cold front.

Water is a very cohesive molecule and will stick to one another when cooled.  This is how hail is produced...

so if you live in a warm area, and a cold front moves in, the air is quick to change temperature faster than the land can.  Hence 90 degree weather and hail.

  2. For the same reason that you can keep ice in a drink outside when it is 90 degrees.  It takes time for the heat energy to melt something so cold.

The hail is formed very high up in the clouds, where the temperature is much lower than at the surface of the earth.  The hail falls so quickly that even the hot temperatures at the surface take time to melt the little ice balls.

  3. Hail  is formed at extremely high altitudes, we are talking 85 thousand feet and more.

Up there it is below freezing, all the time. Same reason why mountain ranges have snow on them.

The hail falls as fast as rocks and gets to the ground before it can thaw out.
