How does my mother show proof that I have had my TDAP shots for 6th grade?

I've had 4 shots for school: TDAP and some others I forgot. (if you know who they are put them in your answer.. I know I had TDAP and Chickenpox..)

My mom didn't get anything from the front desk or the doctors after the shots. How does she show proof to the school I'm going in to that I've had my TDAP shot(s)? (this is apprently required..). Open house is tommorow and I believe that's when they have to have proof.

Call the doctor and ask them to send a copy of your record to the school.  Your mom or dad will have to do it for you.  But either have them send it to your parents, or fax it to the school if you can get a fax number for there.
