  1. You haven't given many details, but typically the metal bed frame will consist of two components. Each of these have a long side and two short sides which fold up into the longer side for storage. Unfold the short sides (they are hinged) on both pieces until they (the short sides) is at a 90 degree angle to the long side. You will now have two components which resemble the sahpe of the letter "C". Lay out the long sides parallel to each other with the short pieces facing each other. As you overlap the matching short sides you should see in the metal frame that one side has a "button hole" like slot. The short side on the opposite side has a "button". Fit the button into the button hole and pull the two long sides in opposite directions to lock the button in the button hole. Do the same on the opposite end. Hopefully the will fit your metal frame arrangement.

  2. The only other type besides the "button" one (which was described perfectly in the previous answers)  is the kind that has a metal clamp that slides onto each pair of short metal pieces. They each have a thumbscrew that you tighten by hand after the two sides are slid close enough to be able to fit tightly around the mattress. That ability to adjust makes them better than the button ones.

  3. "One" would usually assume that a bed frame such as you describe is often sold in two sections.

Those sections would include the long sides with "Hinged/Pinned", fold out ends, with pegs and keyhole slots in the opposing fold outs, meant to be hooked and "locked" in a fashion, allowing the box spring/ mattress to then rest on that frame.
