  1. I imagine that they sprang from the idea that one guy always needs to be in charge.

Whoever could clobber everyone else in order to dominate his local area would become a tribal leader.  The strongest tribal leader became a prince.  The strongest prince became a king, etc.

If you were a king and you knew that your time was up and you needed to pass the leadership to someone else, who better to pick than your own son?  Junior has been hand reared by you and is more trustworthy to your cronies, the same men who have developed vested interests in seeing that things don't change too much.

    Thus, hereditary kingdoms.

  2. Their origin is largely Germanic.  The Germanic peoples inhabited northern / northeastern Europe as Rome spread.  As it weakened, Germanic tribes migrated into the Empire (sometimes by force).  As the Empire disintegrated (ca. 300-500), the tribes supplanted imperial posts, gradually repacing "provincial" authority with the authority of the King (aka tribal chief).  That's very simplified, but it's a start.

These kings established themselves in what had been the West Roman Empire, sometimes as representatives of the East Roman Emperor (as in the case of Pepin the Short), sometimes not.  These kingdoms became what we came to know as England, France, etc.
