It can range from hundreds to thousands of dollars.  The most important thing isn't cost but the quality of the doctor.  Look at their record and see how experienced they are.  References are always good.  Usually higher priced docs are the better ones.

It will usually correct vision completely or even better (20/15).  Lasik isn't for everone, especially if your vision still changes.
