Not that far.  Fuel's expensive, so they don't want to waste it.

I think it's a minimum of 24 miles so they can open the casino and sell alcohol and such with no tax.  But otherwise, unless there's a navigational reason (avoiding shipping lanes, fast currents), after that I believe they just run the engines enough to keep the ship moving smoothly.
