  1. On just about every mil aircraft, chaff and flares are dispensed from a "bucket", and are propelled out. They can be dispensed manually, or automatically by the appropriate system for that particular load. 'Nuff said.

  4. It really depends on the aircraft, some will carry pods that can dispense them and some have them mounted internally.  Flares are normally shot out and away from the aircraft while chaff is normally just dropped out the back.  

    Chaff can either be pre-cut to length or in some cases there are rolls of strands and cutters to cut them to a set, or varied length.

  5. The dispenser, or "bucket" is typically a box recessed into the airframe that contains tubes, like gun barrels, that use a charge of compressed gas to eject a chaff pack or flare. Here are some pictures I've dug up that illustrate different placements. Every aircraft has a slightly different placement, but these will give you an idea of what to look for on others.
