  1. sfr gave you a pretty good answer.  There is one more factor in tread wear, that being your weight.  People who are heavier will wear their back tire more quickly, with everything being the same, as their lighter counterpart.

For maximum life of the tires, when your rear tire begins to wear flat across in the road contact area, rotate the tires.

I get about 2000 to 3000 miles (depending on brand of tire) on road tires.

  2. This is a good question, but there is no way to answer.  It depends on too many factors.  For example, in my experience, some knobbie mountain bike tires used primarilly on dirt last pretty close to forever.  Some soft compound road racing tires are only good for 400-500 miles.  I've gotten almost 3000 miles out of some road bike training tires.  So, you see, it really depends on the type of surface and the type of tire you are discussing.

    Hope this helps.

  3. It depends whether you ride normally or ride downhill or trails, or how often and far you ride and what make they are cos there isn't a set time or distance.
