  3. Individual policies do not have to cover pre-existing conditions. They can have a waiting period of 6 to 12 months (depending on state laws) up to a permanent exclusion or they can decline to accept you alltogether. They will often shorten the waiting period for minor conditions, such as allergies, if you've had previous insurance but they don't have to.

  4. OK, if you get on a group policy, and you didn't have credible coverage for 12 months, the waiting period is usually 12 - 18 months, varies by state.  

    Then, you ask what sort of looks like a second question, "under an individual policy right away".  There's no wait for pre-existing under an individual private policy - because they aren't covered.  So either the condition is excluded, or covered - but usually excluded.  I haven't seen a wait on a private policy.
