Six months to a year.  The physical is comprehensive, including a chest x-ray.  The embassy will ask you all kinds of questions about your fiancee to judge whether it's a real relationship, or if you are just getting married in order to get a visa to the United States.

If you are caught lying, or submit phony papers, you'll never get a visa.
