It depends on where you're starting from in Milan and what the traffic conditions are. Count on 2.5 to 3 hours and maybe somewhat more if there's a lot of traffic. You may also have trouble finding any parking near Portofino. The lots fill up fairly early in the morning.

A better option is to take the train from Milan to Santa Margherita. There's a bus that will take you from there to Portofino. Depending on the type of train, the trip will take about 2 hours to 2 hours 45 minutes. A 2nd class ticket on the faster intercity plus trains is 16.50 euro. The slower regional train is 9.40 euro.
