  4. Jan 27, 1967 - Three astronauts died in Apollo 1 - fire;

    April 1967 - One cosmonaut died in Soyuz I - crash landing, parachute failure;

    1971 - Three cosmonauts died in Soyuz II - valve activated before re-entry, suffocation;

    Jan 28, 1986 - Seven astronauts died in the Challenger - shuttle exploded 73 seconds after takeoff at 46,000ft;

    Feb 1, 2003 - Seven astronauts died in the Columbia - shuttle distintegrated on re-entry due to defective tile on wing edge.

    So far, recorded at the link below, 17 astronauts and 4 cosmonauts have died. There may be more. Too many IMO. But each accident led to changes and advancements, hopefully making it safer for future missions.
