I know for a fact that NBA Live '96 for SNES has Jordan as a unlockable character. All you have to do is choose a "Roster player" and type in Jordan.

I also remember him in NBA Live versions 1998 and 2002 have him in there, though not necessarily under the name Jordan, but his ratings are there.

I think the first attempt was with the 2D game developed by Tecmo in 1992. NBA 2K2 had his image also. NBA Live 2002 and 2003 both featured Jordan in them. The very last game to feature Michael Jordan is NBA Live 2004, The five-year contract originally signed prior to NBA Live 2000's release expired and Jordan chose not to renew. He appears as a member of the 2003 All-Star Team and is removed once a player goes online to update their rosters. Honestly, I don't know the rest.
