  1. You haven't given enough information to provide a completely accurate answer.  

    If you are a US citizen who intends to marry in another country where such a document is required, it is normally a process of only an hour or two to go to the US embassy, complete the document (they provide the blank form, usually), swear to its authenticity and truthfulness, and get the notarial stamps and seals.  You don't need to provide any proof of your single status, since such proof is impossible to get in the US, where marriage is regulated by the states.  However, if you have been married before you should be carrying with you proof of the legal dissolution of all previous marriages.

    The cost is the same as for any notarial service.  Call the embassy, American Citizen Services section, to find out the charge and their hours.
