Technically anything more than the recommended dosage is an overdose. That can be enough to cause permanent damage your liver and kidneys and shut them down in some cases. Painful way to go.

The most common codeine containing products have tylenol (acetaminophen) as a second ingredient. In that case you would more likely die a slow death from liver failure.

Mixed with alcohol, then tylenol is more toxic to the liver, but it also adds to the codeine effects, sometimes people choke on their own vomit, or the just stop breathing.
