How many times did Ben Graham go broke in the stock market?

I don't know for absolute sure, but I would bet it was zero.  Those who go broker are generally speculators, not investors.  Ben was an investor not only an investor but a value investor.  His idea of a good investment was one that had a built in margin of safety.  You would never see Ben investing one cent in First Solar for example, nor I might add overly debt burdened Wall Street investment banks. Heck, he was very unlikely to buy stock in any company that did not pay a dividend.
