  1. That depends on what you mean by "been in". There have been a few major conflicts that we have participated in on a large scale, but there are still more conflicts that we have taken part in but on a smaller scale.

    Cold War Era:

    Korean War, Lebanon Crisis of 1958 (Opertation Blue Bat), Bay of Pigs Invasion, Dominican Intervention (Operation Power Pack), Vietnam War, Tehran Hostage Rescue (Operation eagle Claw), Grenada (Operation Urgent Fury), Beirut bombing, Invasion of Panama(Operation Just Cause).

    Post-Cold War Era:

    Gulf War( Operations Desert Shield and Desert Storm), Somalia (Operation Restore Hope), Yugoslavia (Operation Joint Endeavour.

    War On Terrorism:

    Afghanistan (Operation Enduring Freedom- Afghanistan), Phillipines (Operation Enduring Freedom- Phillipines), Liberia, Iraq.

    There are still other military operations that the U.S. has participated in, but these are the some of the larger and better known ones.

We have only officially declared war one more time - World War II.  The other wars - Vietnam, Korean, 2 Persian Gulf have been "military actions" technically, but if you ask anyone who fought in them they'll tell you the name did not change that it was a war.  The "Cold War" was a period of tension between the US and Russia. We never fought face to face but the Russians were involved in helping the communists in Vietnam and a little bit in Korea.  So your answer could be 5 wars or 1 depending on whether you are meaning when Congress actually approved articles of war.
