  1. There is no need for his code.  All you need do is contact the state insurance commissioner.  Mention the name of the person and the name of the company.  The two characteristics will ID him to the public officials office.

    Be sure you are right.  If you cause him grief and you are incorrect, then (thankfully) the false witness may be sued.  

    If you are willing, consider this:

    Contact the person and / or his company.  Mention your concerns.  Ask for input from each.  A problem may be resolved.
