  1. I think you should use the number of hours to finish the job multiply by your desired rate per hours.  If you have more than one worker with the same hourly rates just multilply it by 2.

  2. Is that 30 man hours (2@15ea), or 15 total?  For sash windows, 1.5-3 hours isn't unreasonable, depending on number of panes and how rough a condition they were in.  For casement or sliders, maybe 1-1.5hr/ea for inside and out with trim/moulding and rough shape.

Can't really give a per piece price without more detail (window type, size, trim, condition, etc).  But on a time basis, a contractor might bid the job at $30/hr, and pay their labor $15.  If your time was inline with the above, maybe charge $20/hr.  If you were slower, maybe $15/hr

  3. People are hiring you without knowing what they'll pay you? That's something you discuss before you start the job. Better for both sides that way. You can charge by the job or by the hour, rates vary from place to place so you have to find out what is usually charged where you live. I charge $15/hour right now for painting, though that will change soon. My friend, in a more urban area, charges $25/hr
