  1. here in the US it's typically around 250 - 500 depending on the cloth.  Championship titan, and/or teflon will be the cheaper side, while Simonis, and Championship Tour cloths will be more around 400-500 depending.  You should have them relevel the table when it's reclothed, most places include this as part of the service.  It will also depend on the type of cloth, Simonis makes a few different kinds as well, there's a traditional 8 and 9 ball cloth, the 860, but there's also the 760 which is a little faster, and the 300 rapide which is faster still, the latter two are intended for snooker and carom billiards where it's traditionally a much larger table.  Best bet, call around to a few local places and get some prices, don't just go for the lowest, ask how long they've been doing it, and make sure they're familiar with both glue down apps as well as stapled cloths, if you know which you need it helps them.  And the type of application can make a difference too, so keep that in mind.

It depends on the cloth (I like simonis 860).  Also who you use to do it.  Don't go automatically for the cheapest price because stretching the cloth and recovering the rails properly can make a huge difference in the way your table plays.  I would call your local Pool Table distributor there - They should be able to recomend someone good. Also your local pool rooms usually have someone they use.
