  1. you'll need your health card, ssn,


  2. It should be covered by your provincial health insurance and possibly any supplementary insurance you have. Take proof of insurance with you and if in doubt, phone the doctor's office before your appointment.

  3. You need a referral from your family doctor. Then you need your OHIP card.  It shouldn't cost you anything.
