Who could I call to find out? Likewise, how do I find out for private insurance companies?

For private insurers, all you need do is call one of us.  We would be able to get the answer to you in minutes or days, depending upon the policy and other factors.

For the government, why not call your local county health department?  If they cannot tell you, then ask for the phone number of the Social Security Administration or other local office that handles Medicaid.
